Reducing risk on the labour ward

This is an exciting time to be involved in risk management on the labour ward as the focus is shifting from litigation prevention to that of reducing patient harm. This review concentrates on the practical aspects of implementing effective risk management on the labour ward. We need to move from a person approach aimed at the active failures of the obstetricians, midwives and anaesthetists on the frontline of the labour ward towards a system approach. This will identify the error and violation producing conditions and the managerial and organisational processes that give rise to the active failures.
